It seems to be made by turning Squid Head's bonce upside-down and painting eyes on those things on the back of his head that look like bollocks.

Mongo Beefhead. The only creature in the galaxy whose eyes are below his mouth. And look a bit like bollocks.

In a supermarket the other day I saw figure for a character from Return of the Jedi that I'd never heard of or even recalled seeing him in the film:
